The following are selected letters of investigative findings issued in the current fiscal year (October 1, 2008 – September 30, 2009) and additional selected letters from prior years.  Additional historical documents will be added in the future, as well as more recent documents as they become publicly available.

The findings concerning the conditions set forth in findings letters relate only to the quality of care provided by the jurisdiction. These findings do not in any way reflect upon the care provided to residents by their families. The Department has the deepest appreciation, and respect for, those families who visit and care for their loved ones at facilities addressed in our findings letters.
